RAI-MH

The Resident Assessment Instrument for Mental Health is a comprehensive, standardized assessment tool used to collect the information and data required to evaluate the needs, strengths, and preferences of our clients, as we plan for their care. Here you will find information about upcoming training sessions, resources and information on the Resident Assessment Instrument for Mental Health (RAI-MH) and its components.

The RAI-MH tool has 2 main components:
 
1. The Minimum Data Set –Mental Heath (MDS-MH) 
This is a standardized minimum assessment tool that has been developed and is being used by our nursing and clinical staff to describe client performance and capacity. The items in this instrument are useful in assessing a wide variety of client populations and add structure to our clinical interviews, our team conferences and therapeutic interventions. Our goal is not just to promote but also assist our clients in their recovery. 
 
2. The Mental Health Assessment Protocols (MHAPs)
The MHAPs are the general guidelines for care planning that are developed directly from the assessment data we collect. These protocols identify the individual needs and many of the specific issues that our clients and their families are coping with. They also help us to identify our client’s strengths and the resources that are available to them, so that our treatment teams have the information they need for planning recovery focused interventions that are appropriate and effective, for each individual.
