Located at St. Mary's of the Lake Hospital site, this program offers individualized assessment, injury-specific treatment, functionally oriented exercises, education, and prevention with the aim of restoring persons to their pre-accident status. The treatment team includes physiotherapy, occupational therapy, psychology, and social work.
Eligibility: For persons who have been injured in a motor vehicle accident, including muscoloskeletal injuries, spinal cord injury, vestibular disorders, and amputees.
Waiting period: 5 Working days.
Fees: Covered by auto insurance carriers.
Referrals: A written physician's referral is required. The injured person must notify their insurance company within 30 days of the accident.

Located at St. Mary's of the Lake Hospital site, this Occupational Therapy Service is designed to optimize independence in activities of daily living including self-care, leisure, homemaking tasks, and shopping. Therapists make recommendations for equipment and adaptive devices, and educate the client and significant others.
Eligibility: For individuals with any musculoskeletal or neurological dysfunction that affects their ability to manage activities of daily living.
Waiting period: 1 - 2 weeks for patients with neurological conditions; 2 - 3 months for general outpatients.
Fees: Covered by OHIP or extended health insurers.

Located at St. Mary's of the Lake Hospital site, the clinic provides physiotherapy intervention with an emphasis on education and active rehabilitation to return the injured worker to work as safely and efficiently as possible.
Eligibility: Any musculoskeletal injury sustained at work.
Waiting period: Within 5 working days if referral is within 70 days of the injury; if the referral is beyond 70 days of the injury the client will be seen as soon as possible, usually within 4 weeks.
Fees: Covered by WSIB. The attending physician must complete WSIB form #8 (Physician's First Report) and forward it to WSIB directly to initiate the process of claim.

Located at St. Mary's of the Lake Hospital site, the Evaluation Centre provides multidisciplinary assessments of injured workers by a physician, physiotherapist and, if necessary, a social worker or psychologist to determine the need for further investigation or treatment, and to make recommendations regarding return to work.
Eligibility: For persons who have experienced a work-related injury over 8 weeks ago but less than one year ago. Exceptions to this time line must be pre-approved by WSIB.
Waiting period: 10 working days.
Fees: Covered by WSIB.

Eligibility: Any musculoskeletal condition.

Provides physiotherapy intervention to improve strength, motor control and balance, and to increase functional mobility.
Eligibility: Any neurological condition.
Waiting period: 2 weeks for outpatients with an acute neurological problem;
6 weeks for non-priority situations.

This clinic provides an assessment of the consequences, severity, and complications of an acquired brain injury and makes referrals for treatment to therapists and other agencies as appropriate.
Eligibility: For adults with an acquired brain injury of any kind (traumatic or otherwise).
Waiting period: ~ 6 months.

This clinic provides assessment and treatment for focal spasticity due to injury or disease of motor neurons.
Eligibility: For all ages with spasticity resulting in functional limitations.
Waiting period: 4 months.

This clinic provides assessment of neural and muscle function by the use of electrical analysis of hological waveforms.
Eligibility: All ages.

This clinic provides assesment of neuromuscular diseases (eg ALS, DMD, ataxic telangiectasia etc.)

This clinic provides assessment of individuals who are experiencing shortness of breath due to lung disease to determine their eligibility for participation in the Respiratory Rehabilitation Program. If this program is deemed inappropriate, alternate recommendations will be made to the referral source.
Eligibility: For individuals experiencing shortness of breath due to Chronic Obstructive Pulmonary Disease, asthma, lung disease, or lung surgery (pre & post).

This clinic provides assessment and management of stroke related impairment, disability and complications for adult patients.

This is a comprehensive cardiorespiratory fitness and education program. Outpatients participate in an exercise program three afternoons a week from 1 to 4p.m., and also attend educational sessions regarding lung disease and its management.
Inpatient Program: Two beds are available at St. Mary's for individuals who live too far away to drive to the program three times a week. The inpatient program involves three weeks in hospital (with weekend passes), and an intensive exercise program of 2 - 2 ½ hours both morning and afternoon five days a week.
Location: St. Mary's of the Lake Hospital site, physiotherapy gym.
Eligibility: For people experiencing shortness of breath due to lung disease, but who have no cardiac or orthopedic problems that preclude intensive exercise. Patients are assessed by the Respirology Clinic as to their suitability for this program.
Waiting period: 3 months.
Duration: Generally 8 weeks for outpatients; 3 weeks for inpatients.
Follow-up: For both outpatients and inpatients, follow-up is offered at 2 months, 6 months and 1 year after completion of the program. A home exercise program is prescribed. Patients can also join the Breathe Easy Club, which meets on Thursday afternoons, for ongoing maintenance and support.

Located at St. Mary's of the Lake Hospital site, this service provides communication assessment, therapy, education and counselling with the goal of the client becoming a functional communicator in his or her environment.
Eligibility: For persons with any acquired neurogenic communication disorder.
Waiting period: As soon as possible for priority situations (i.e. stroke and aphasia); 6 - 8 weeks for non-priority.
Fees: Covered by OHIP or extended health insurers.

A support and educational group is held monthly for stroke survivors and spouses. A Rheumatoid Arthritis Group meets four times yearly. The groups are facilitated by a social worker.
Location: St. Mary's of the Lake Hospital site.
Eligibility: Participants must have a diagnosis or situation in keeping with the group focus.
Waiting period: None. A new referral can join the next group session.

Located at St. Mary's of the Lake Hospital site, this service educates patients about balance and their particular condition, and provides physiotherapy intervention to improve balance, dizziness, and overall level of function.
Eligibility: For patients with peripheral hypofunction, bilateral vestibular loss, benign paroxysmal positional vertigo, symptomatic Meniere's, post traumatic vertigo, cerebellar deficits, central vestibular dysfunction.
Waiting period: 2 weeks.
Fees: Covered by OHIP.

This service provides assessments and interventions to help individuals return to work after an injury or a medical condition.
Occupational Therapy Services
Occupational therapy enables people to lead productive, independent and satisfying lives by assisting them to set goals with self-care, (e.g. dressing, eating and shopping), household management, as well as activities that allow them to work or enjoy free time. During an assessment, the occupational therapist identifies which activities are difficult and why by checking physical abilities (strength, coordination, balance), mental abilities (memory, organization, coping), level of support available, and environmental barriers (at home, school, workplace or community). Intervention may then involve learning new ways of doing things, using adaptive equipment and devices, making changes to the environment, or exploring new interests and developing new skills.

Vocational Rehabilitation Counsellor Services
Vocational Rehabilitation is a component of the continuous and coordinated process of rehabilitation involving services such as vocational assessment, training, education and selected job placement to enable persons’ with disabilities to secure and retain meaningful paid or non-paid employment. Rehabilitation Counsellors have strong knowledge of vocational assessment and career development, medical aspects of disability and adjustment counselling and are well suited to provide services such as, coordinating catastrophic case management, preparing life care plans/future care cost reports, assisting individuals in developing and implementing return to work plans, conducting vocational assessments, determining loss of earnings capacity, placement coordination and expert testimony. Services are individualized and tailored to meet the needs of the client to maximize employability, independence, and community integration.
