Community Connections is a community-based, individualized rehabilitation program that offers a supportive environment where clients can learn skills and coping strategies necessary for successful community living. The program helps clients to regain valued roles and to continue along the path to their own recovery.
- Morning program: focuses on establishing productive roles and routines; improving cognitive skills; learning new skills e.g. cooking, money management, computers, meal planning and preparation; and socializing with new friends
- Afternoon program: uses a variety of treatment approaches including one-to-one interventions, psychoeducation, personal exploration and skill enhancement
- Support from staff who model self care, healthy living, competence, self worth, flexibility, and values of trust and hope.
Hours
Mondays to Thursdays, 8:30a.m. - 3:00 p.m.
Hours of attendance are flexible.
Referral:
Interested clients should speak to their health care professional or contact Community Connections directly. Upon referral, you will meet with an occupational therapist to assess whether the program can meet your needs and goals.
