Assertive Community Treatment (ACT) Teams


The Providence Care Schizophrenia Rehabilitation Service provides intensive treatment, rehabilitation and support to adults living in the Kingston area through two Assertive Community Treatment (ACT) teams:
  • Psychosocial Rehabilitation ACT Team
  • Community Integration Program ACT Team.

Most clients are diagnosed with schizophrenia, have a history of frequent hospitalizations and may continue to experience functional impairment (e.g. in self-care, work, and relationships).  The objective of both teams is to provide recovery oriented services that promote hope, empowerment, self determination and wellness.

  • medications provided daily or as needed
  • individual psychoeducation, skills teaching, vocational, and recreational support
  • 24-hour on-call support collaboration with agencies and families.
